Referrals will need to be accompanied by a full Medical Report. This much contain the following information:
The Medical Report should provide full details of the patient's present condition, relevant medical history, provisional diagnosis and the treatment or investigations expected:
The Medical Report:
- must clearly identify the patient it describes
- must be less than three months old
- must be written in English
- must be written by the patient's attending physician only on the hospital's letterhead paper
- must be stamped with the hospital's official stamp
- must include the doctor's fax /phone /email details
